Charlotte Tavakoli, MHSA, RN
Director of Nursing and Clinical Practice

In 2025, we celebrated the incredible resilience, creativity, and collaboration of our hospital and ambulatory care teams. Together, we continued to raise the bar for nursing excellence, improving the quality of care and making a real difference for our patients.

Our Emergency Department showed team improvement within triage workflows to shorten wait times, reducing patient falls, strengthen our sepsis and stroke pathways through evidence-based protocols, and increased participation in hospital-wide committees to support teamwork across departments.

Interventional Radiology continued to shine as a leader in collaboration and nurse-driven improvement. IR nurses are playing a key role in shared governance councils and bringing forward ideas to enhance safety, streamline procedures, and improve communication between teams. Their active participation will continue to drive system-wide improvement and reinforce a culture of professional growth.

Our Cardiac Catheterization Lab also reached new milestones this year. Nursing was a key component as we introduced an electrophysiology lab allowing advanced imaging technology, refined recovery protocols to help patients go home sooner, and partners closely with ED and IR teams to strengthen cardiac care pathways.

The expansion of our Urgent Care and opening our new Ambulatory Treatment Center highlights out continued commitment to patient-centered care and operational excellence. These achievements are the result of dedication, compassion and teamwork shown by ever nurse and nurse leader across Kaiser Permanente San Francisco.

To all our nurses, thank you. Your skills, kindness, and determination inspire us all. Together, we are shaping the future of nursing and delivery at San Francisco that truly reflects our mission to improve the lives of our members and their families.
