Samantha “Sam” Avecilla, MSN, RN, CRRN, CNL, demonstrates how systematic community engagement can address discharge planning inefficiencies that plague contemporary healthcare delivery systems. Her progression from Association of Rehabilitation Nurses (ARN) member two years ago as a frontline nurse, through Continuity of Care (COC) case manager, to Kaiser Foundation Regional Center management in August 2025, positioned her to identify a fundamental operational disconnect between institutional care and community resource utilization.
Case management protocols traditionally terminate at discharge, creating care continuity gaps that disproportionately affect vulnerable populations. Sam’s dual role as ARN Vice President revealed how structured community involvement could function as clinical intelligence gathering rather than peripheral volunteer activity.
Dignity Day operations occur biannually each second Saturday in April and September, partnering with Solano County churches to deliver comprehensive services to Vallejo’s unhoused population. Clinical services include hygiene access, grooming assistance, clothing distribution, nutrition support, medical screening, and housing assistance coordination. However, the measurable impact extends beyond immediate service delivery.
“I genuinely enjoy this work. It fills my cup knowing what resources exist in our community,” Avecilla explains. “I collect brochures and information so I can share them with our team and connect patients who need these services.” This systematic resource documentation directly enhances case management effectiveness for Medi-Cal patients requiring community-based transitional support.
The model challenges established boundaries between clinical practice and community health infrastructure. Current discharge planning protocols assume community resource knowledge exists among healthcare teams, yet evidence suggests otherwise. Sam’s approach quantifies available community assets, establishes provider relationships, and documents service gaps. Creating actionable intelligence that improves patient outcomes.
