The ICU pilot project, developed by the UPC committee, was powered by the unwavering commitment of our nursing team to streamline care coordination and elevate the patient experience. By assigning a dedicated PCC to the entire ICU, our nurses not only ensured regulatory compliance but also brought compassion and consistency to every patient interaction.
Before this project, ICU patients often faced inconsistent coverage, missed assessments, and delays in care. Thanks to the nurses’ leadership and advocacy, these challenges were transformed into opportunities for excellence.
