Empowering nurses, enhancing care: The Informatics Magnet story

The Informatics Magnet Council is bridging technology and bedside care to enhance safety and efficiency across the Kaiser Permanente Fremont Medical Center. In 2025, the council launched education sessions to strengthen staff readiness for system downtimes by creating poster presentations and resources for individual departments. By ensuring every nurse knows exactly how to document and access critical information when electronic systems are offline, we can help safeguard patient care even in unexpected situations.

Building on that foundation, the council is spearheading an innovative KP HealthConnect (KPHC) project to improve shift-change report amongst nurses. Using existing tools within KPHC, nurses can share patient updates in a clearer, more standardized format, reducing redundancy and missed details. The goal is a streamlined handoff that improves communication between caregivers and gives patients a smoother, safer experience.

Through collaboration, training, and a focus on practical solutions, the Informatics Magnet Council is showing how technology can empower nurses—not replace them. By connecting people, processes, and data, we’re setting a new standard for communication and patient-centered care throughout our organization.
