Reducing sepsis readmissions

Antioch Medical Center’s Research and Evidence-Based Practice Core Council is tackling sepsis readmissions head-on!

Our Evidence-Based Practice (EBP) Core Council selected sepsis readmissions as the focus for facility-wide impact. Facility data indicated that sepsis was a leading causes of hospital readmissions at the Antioch Medical Center. In alignment with our organization’s strategic priorities to enhance quality, safety, and patient-centered care, our core council is implementing evidence-based interventions utilizing EBP methods we have learned and strengthened from the training we received throughout the year. This project aims to reduce readmission rate for our telemetry units by 3 percent from September 2025 through November 2025.  

Our interventions are intended to strengthen post-discharge follow-up and improve the effectiveness of patient education upon discharge for our patients with sepsis diagnosis.  By prioritizing these strategies, we seek to support better patient outcomes, reduce the risk of complications, and ensure continuity of care beyond hospitalization. This project reflects our commitment to translating evidence into practice to drive meaningful improvements in care.
