At Kaiser Permanente San Jose, protecting our patients from harm is at the heart of everything we do. Our Hospital-Acquired Pressure Injury (HAPI) Prevention Program brings together a dedicated team of leaders, nurses, educators, providers, and quality experts, all working toward one shared goal: keeping our patients safe and promoting healing.
A key part of this effort is the partnership between our Wound, Ostomy, and Continence (WOCN) nurses and our frontline care teams. These highly skilled nurses serve as both educators and advocates, helping staff identify risks early, implement prevention strategies, and ensure every patient receives the highest standard of care.
Our WOCN team describes their work best: Together, we’ve built a culture of prevention, one focused on early detection, teamwork, and patient-centered care to minimize pressure injuries and support healing.
Recent efforts have focused on comprehensive staff education, advanced wound care training, and the use of innovative products that protect skin integrity. By enhancing early risk recognition, reinforcing repositioning and skin care practices, and introducing moisture-wicking dressings and protective devices, our nurses are empowered to act quickly and confidently.
It’s inspiring to be part of a changing culture, one that strengthens both patient care and nursing practice. With strong leadership support and the right tools, we’re turning the focus back to what matters most: our patients.
Through collaboration, compassion, and continuous improvement, our teams are making a meaningful difference every day, ensuring that every patient entrusted to our care is protected, respected, and supported on their healing journey.
