Journey to zero harm

KP San Leandro’s Annual Quality & Safety Faire Showcases Best Practices and Honors Top Performers.

The Annual Quality & Safety Fair, held in March 2025, embraced the theme “Journey to Zero Harm.” Clinical Quality Director, Alma Domingo MSQA, BSN, RN, CPHQ, CLSSBB, organized this event as a platform to showcase best practices for preventing patient harm. Working with her team of quality nurses, she identified hospital teams that consistently achieved outstanding quality outcomes and invited them to share their strategies. She also reached out to teams that demonstrated significant improvement in turning around quality metrics, ensuring their success stories were highlighted.

Because the quality nursing team is deeply involved in developing and supporting improvement processes, they knew exactly which teams to feature. Posters presented at the fair included:

  • ICU: Glucose Management Best Practices
  • ICU: Chronic Heart Failure Education
  • NICU: Retinopathy of Prematurity Best Practices
  • Infection Control: C-Diff Prevention through Proper Hand Hygiene
  • Perioperative Units: Strategies for Preventing Surgical Site Infections

The “Journey to Zero Harm” Quality & Safety Fair also celebrated excellence with special recognitions: NICU was honored as “Guardians of Patient Safety” for achieving the lowest composite harm score at KP San Leandro and earning first place for their poster along with perioperative units recognized for the greatest reduction in composite harm scores from 2023–2024.
