CCU’s winning strategy against C-diff features bold visuals and smart bundles

A proactive workflow and standardized tools empower Critical Care Unit nurses to set a new infection prevention and patient safety standard.

In 2025, the Critical Care Unit (CCU) nursing team identified a critical gap in infection control: patients in isolation were often difficult to recognize because isolation signs hung on sliding glass doors that frequently shifted out of view. This made it easy for staff or visitors to enter rooms without proper PPE, increasing the risk of Clostridioides difficile (C-diff) transmission.

Recognizing the urgency, the CCU Unit-Based Council (UBC) launched a quality improvement project grounded in evidence-based practice. The team introduced a bold, highly visible solution – a large, brightly colored floor sticker placed at the room’s entrance. This visual cue ensured anyone approaching the room immediately recognized isolation precautions, even if door signs were obscured.

To streamline care and reduce delays, nurses also developed a comprehensive C-diff Bundle Kit stored in a centralized location. Each kit included everything needed to test and manage a patient under C-diff rule-out: the floor sticker, specimen collection cup and scooper, biohazard bag, required lab documentation, and both “STOP” and “Contact Plus Isolation” door signs.

By standardizing this process, the CCU transformed infection prevention into a proactive, efficient workflow. The results were remarkable: zero C-diff cases in 2025 within the CCU, contributing significantly to the hospital’s overall reduction in C-diff rates. This project exemplifies how frontline nurses, empowered by evidence-based strategies, can drive meaningful change and improve patient safety.
