Nurses at the Walk to End Alzheimers

Nurses join the Walk to End Alzheimer’s

Several nurses from Adult Inpatient Services participated in community service and outreach through participation in the Walk to End Alzheimer’s. At the event, we offered information about advanced directives, fall prevention at home, medication safety, and delirium. These topics are in alignment with the Institute for Healthcare Improvement Age-Friendly initiative that focuses on the 4 M’s – What Matters Most, Mobility, Medication, and Mentation.

Fostering a continuous learning culture through peer feedback

Registered nurse peer feedback is the hallmark of a nurse’s professional practice. According to the American Nurse Association, it is the nurse’s professional obligation to maintain competence and contribute to their own personal and professional growth.

“Peer feedback is practice focused, fosters continuous learning as it relates to patient safety and best practices, and is done by all levels of registered nurses,” said Janet Sohal, DNP, RN, NEA-BC, Regional Director, Kaiser Permanente Nurse Scholars Academy & Magnet.

In 2024, Kaiser Permanente Northern California implemented an electronic platform known as KP Mentoring, that allows the nurse to send and receive feedback from their peers. Nurses can incorporate the feedback they’ve received as they complete their self-appraisal and develop a professional goal for the year at the time of their annual review.

Mimi Denila, BSN, RN, a staff nurse II at the Vallejo Medical Center says, “Peer feedback tools have been helpful by holding each other accountable and ensuring patient safety.”

The peer feedback process is not new, in fact, nurses provide feedback continually to each other, including during patient hand offs, when assisting with a procedure, planning the care for the patient, when working on quality initiatives, and during formal or informal teaching activities.

“As we move forward in 2025, we will continue to encourage our nurses to send and receive peer feedback,” added Sohal. “Feedback is a gift for their professional growth and development and fuels the essence of excellence in nursing for our patients and families that we serve.”

Walnut Creek Councils

Core Council Chairs

Thomas Booth, BSN, RN, PCCN
Chair, Magnet Ambassadors

“This year, our council made significant progress in fostering a supportive environment for nurses and health care professionals. In 2024, we proudly formed the Magnet Ambassadors, who played a key role in supporting the RN Excellence Survey to gather valuable feedback on nurses’ experiences and practices. Additionally, we collaborated with administration to successfully plan and execute a joint Nurses and Hospitals Week, celebrating the dedication and hard work of our health care team.”

Maria Cabrera, ADN, RN
Chair, Professional Development Core Council

“In 2024, the Professional Development Council prioritized membership growth and conducted a comprehensive house-wide survey to inform our strategic focus for 2025.”

Gladys Miller, BSN, RN, PHN
Chair, Quality and Safety Core Council

“The Nursing Professional Governance now includes the Quality and Safety Council. The Quality and Safety co-chairs include Falls, HAP, CAUTI, CLABSI, HAPI, and C-Diff. A few of the accomplishments include updating and completing the charter. We have trained the co-chairs on data review resources, how to use an A3, and how to run a successful meeting. This council was established to ensure a system of shared decision-making and accountability between all levels of staff.”

Zahra “Minoo” Nouri, BSN, RN, CEN
Chair, EBP, Innovation, and Research Core Council

“In October, we attended the Magnet Conference, where we explored various posters and evidence-based practice (EBP) projects from hospitals nationwide, including notable initiatives by Northern California Kaiser Permanente. I participated in the KP/UCSF EBP Strategic Planning Session as part of a group. This session will serve as our guiding framework as we move into 2025.Under the guidance of Gilbert, we formed a council comprising Zahra Minoo Nouri (Chair), Alex Fogli (CNS), and Autumn Argent (Nursing Advisor). We have successfully recruited six additional members and are ready to begin advancing EBP Initiatives.”

Abigail Velez, ADN, RN
Chair, Care Experience and Caring Science Core Council

“To enhance patient care, we integrated the Care Experience and Caring Science Core Council into Nursing Professional Governance. This new structure and support system facilitated an Evidence-Based Practice (EBP) project involving multiple Plan-Do-Study-Act (PDSA) cycles. This project centered on optimizing patient goal setting through the effective use of communication boards. Concurrently, we initiated another EBP project aimed at implementing updated care boards across all Adult-Patient Care Services (Adult-PCS). We are immensely proud of these achievements, as they signify a significant step forward in improving the quality of care we provide to our members.”

Feed the Hungry program

The Feed the Hungry program started initially on the 3OB unit. Jen Dash, BSN, RN, SN IV jump started this program when she identified the amount of waste that was taking place each shift. She advocated and worked with the nutrition director at the time to develop a workflow that included trays from the previous shift being placed in a bin at the bottom of the patient refrigerator if they were not used by patients the previous day.

This made an easy workflow for the nutrition team to pick up and bring it back to their department to get ready for donation. Once the process was streamlined, Jen helped get it started on the 6OB unit as well. We currently provide approximately 5-10 meals a day to: Replate, Epiphany House, and Food Runners within the city of SF. Since it has been such a large success, Jen is helping to move this project hospital wide hoping to make even more of an impact within the community. In total, this project has donated approx. 2,000 meals to local San Francisco non-profit organizations.
