This dedicated discharge nurse works closely with care teams to streamline the discharge process, ensuring timely education, coordination, and communication with patients and families. Early results of the pilot program have highlighted improved efficiency, reduced delays, and enhanced patient satisfaction, showcasing the power of collaboration in driving organizational
