Quality and Safety Council implements Fall TIPS Toolkit to strengthen patient fall prevention

Nurses turn evidence into action as Fall TIPS brings personalized fall prevention to every bedside at KP SSF.

In late November 2025, the Kaiser Permanente South San Francisco (KP SSF) Quality and Safety Council proudly began implementing the evidence-based Fall TIPS (Tailoring Interventions for Patient Safety) Toolkit, a proactive, innovative approach designed to prevent patient falls and falls with injury across inpatient units. Recognizing that falls remain among the most common and preventable hospital safety events, the council, composed of staff RNs, worked collaboratively to bring this national best practice to life at KP SSF.

The Fall TIPS Toolkit bridges the gap between patient assessment and action by translating individualized fall risk factors into visible, easy-to-understand prevention plans at every bedside. Inpatient nurses identify each patient’s specific fall risks, such as impaired mobility, medications, or toileting needs, and generate a personalized poster outlining tailored interventions. These bedside visuals serve as a daily reminder for patients, families, and care teams, ensuring that fall prevention strategies are clear, consistent, and patient-centered.

Peer-led education sessions and hands-on demonstrations were conducted throughout November to prepare nurses for the rollout. The Quality and Safety Council, supported by nursing leadership, provided unit-based resources and guidance to help teams incorporate the toolkit into daily care routines. Early feedback from nurses has been overwhelmingly positive, highlighting how the posters enhance communication, engage patients in their safety, and increase team accountability.

Implementation of the Fall TIPS Toolkit aligns with KP SSF’s ongoing commitment to delivering safe, high-quality, and evidence-based care. In 2026, the Quality and Safety Council will continue to monitor outcomes using NDNQI fall and fall-with-injury data and will use these insights to further strengthen safety practices. Through collaboration, education, and innovation, KP SSF nurses continue to lead the way in creating a safer healing environment for every patient.
