The journey home: Our story

Our goal was to help patients travel on the continuum from admission to discharge.

With a goal to support patients throughout their journey from admission to discharge, we asked a simple but important question: How can we help patients get home sooner while feeling informed and confident about the process?

Guided by evidence on visual management, the team developed the “Road to Discharge Board Checklist.” Research shows that when patients can clearly see their progress and understand potential barriers, satisfaction improves and discharge delays are reduced. To ensure consistent adoption, we created a short video for staff meetings that demonstrated how this tool transforms a routine checklist into a meaningful, patient‑centered conversation.

The pilot launched on April 13, 2025, with checklists placed in patient rooms upon admission. Nurses updated the boards as key milestones were achieved, turning the tool into a shared roadmap between patients and the care team—not just a form, but a guide to recovery.

The results were compelling. Post‑implementation data showed increased patient satisfaction alongside a reduction in length of stay. More timely discharges improved room turnover and supported new admissions. This project demonstrated that clarity drives results, and we look forward to expanding the checklist across the Walnut Creek facility so more patients can confidently navigate their road to recovery.
