Fresno team celebrates over a year of zero CAUTIs

Care teams achieve extraordinary patient safety success by embracing High Reliability Organization principles and consistent standard work

At Kaiser Permanente Fresno, patient safety isn’t just a priority – it’s a promise. Our nurses and care teams are committed to delivering the highest standards of care, and their dedication has led to an extraordinary achievement: As of December 1, 2025, it has been 439 days since the last catheter-associated urinary tract infection (CAUTI).

In June 2025, Kaiser Permanente Fresno was recognized by Becker’s Hospital Review as one of the hospitals with the lowest CAUTI rates nationwide — a distinction that speaks volumes about the team’s unwavering commitment. These milestones represent more than just numbers; they represent protected lives.

This success is the result of consistent standard work, adherence to Daily Management System (DMS) principles, and embracing High Reliability Organization (HRO) practices. These frameworks ensure accountability, consistency, and continuous performance improvement. This is work is evidenced by the completion of daily catheter audits and ongoing education provided to the nursing staff.

Yet, the true driving force behind this achievement is the diligence of our nurses and care teams. Every catheter insertion, every assessment, every patient conversation is guided by a shared mission: Safety first.
