In the first quarter of 2025, an interdisciplinary team at Kaiser Permanente Vacaville Medical Center including clinical RNs from the 3A/3B medical-surgical units, Patient Care Coordinator–Case Managers (PCC-CM), leadership, and physicians developed a discharge milestone tool as part of the Comprehensive Discharge Value Stream Analysis Project. This initiative introduced milestone tracking to improve care coordination, streamline workflows, and enhance the patient-centered discharge process.
By August 2025, Vacaville Medical Center advanced the effort with a more robust, team-based discharge planning model that begins at admission. Streamlined workflows, clearly defined roles, and technology integration now reduce delays, remove barriers, and improve the discharge experience for patients, families, and staff.
Key interventions include enhancing the morning discharge huddle with milestone tracking, adding an afternoon huddle for loop closures, and establishing clear milestone steps to discharge. Bedside shift reports now incorporate “hover and discover” milestones, while redesigned patient care boards reflect frontline expertise and provide a visual roadmap of the discharge process. These improvements have strengthened communication, supported timely decision-making, and reduced discharge turnaround times by 17% as of December 2025.
The best practices developed at Vacaville will serve as a blueprint for scaling across all Northern California Kaiser Permanente medical centers, ensuring smoother, more predictable discharges for members and families.
