6North Med/Tele team achieves 325 days of zero patient harm

Collaboration, education, and shared learning drive an extraordinary quality and safety milestone.

Kaiser Permanente Redwood City’s 6North Med/Tele team celebrated an incredible achievement: 325 consecutive days of Zero Patient Harm on March 16, 2025. This means no CAUTI, CLABSI, HAP, HAPI, CDI, falls with injury, or workplace injury; a testament to the team’s commitment to patient safety and quality care.

This success didn’t happen by chance. It reflects a culture of collaboration and continuous improvement, supported by several key strategies:

  • Peer-to-peer education on the units, ensuring staff share best practices in innovative ways and reinforce safety protocols.
  • Active participation in the HEROES Committee and Falls Taskforce, where every near miss and incident is openly shared across units.
  • A system-wide approach to learning from each other, turning challenges into opportunities for improvement and preventing harm before it occurs.
