Kaiser Permanente Redwood City’s 6North Med/Tele team celebrated an incredible achievement: 325 consecutive days of Zero Patient Harm on March 16, 2025. This means no CAUTI, CLABSI, HAP, HAPI, CDI, falls with injury, or workplace injury; a testament to the team’s commitment to patient safety and quality care.
This success didn’t happen by chance. It reflects a culture of collaboration and continuous improvement, supported by several key strategies:
- Peer-to-peer education on the units, ensuring staff share best practices in innovative ways and reinforce safety protocols.
- Active participation in the HEROES Committee and Falls Taskforce, where every near miss and incident is openly shared across units.
- A system-wide approach to learning from each other, turning challenges into opportunities for improvement and preventing harm before it occurs.
Through these efforts, the 6North team exemplifies what’s possible when safety is everyone’s responsibility. Congratulations to every team member for making this milestone a reality, and for setting the bar high for patient care excellence across the organization.
